Suppliers that want to be pre-qualified in Connexio (stage 1) and to qualify for the participating utilities (stage 2) have to provide information about the below mentioned areas. The standard questionnaire is not completed by suppliers that selected the "Basic Registration". The declaration signed by the managing director confirms the accuracy of the given information in the questionnaire.

General information about the company such as company name, address, phone numbers, Homepage etc.
- Personnel Information
Information about contact person and management such as managing director, Technical Director, Sales Director, Financial Director as well as number of employees
- Legal Information
General information such as year of registration, company registration number etc. as well as information about owners/shareholders and financial auditor/head of accounting
- Insurances
Information if the company has certain insurances and what damages are covered by the insurances.
- Associated Companies
For example parent company and other associated companies
- Financial Information
Information such as financial key figures of the last 3 years, regional distribution of turnover, investments in R&D
- Quality Management
Information about the company's Quality Management System, Quality Manager etc.
- Environmental Management
Information about the company's Environmental Management, Environmental System Manager etc.
- Corporate Social Responsibility (CSR)

- Products and/or Services
Detailled information for each product and/or service: description either in English, German or Slovak, company’s role in supplying the product/service, area of sale, partner/sub-contractors, references, contact person of the product/service
